1. About the FL C.A.R.E. Project

The FL C.A.R.E. Project (Collect, Assess, Register, Educate) is a public health awareness initiative operated by CFL Water Treatment in DeLand, Florida. Our mission is to build Florida's first independent household water quality registry and educate Florida families about water quality issues.

We are not affiliated with any federal, state, or local government agency.

2. Registration & Services

By submitting the registration form at flcareproject.com, you:

• Voluntarily register your home in the FL C.A.R.E. water quality registry
• Consent to be contacted by a C.A.R.E. field analyst to schedule a no-cost in-home water assessment
• Consent to receive SMS text messages and emails from the FL C.A.R.E. Project regarding your registration and assessment
• Agree that the information you provide is accurate to the best of your knowledge

The in-home water assessment is provided at no cost and with no obligation to purchase any product or service.

3. SMS Terms

By registering, you consent to receive recurring SMS text messages from the FL C.A.R.E. Project at the mobile number provided. Messages may include:

• Registration confirmations
• Appointment scheduling and reminders
• Follow-up communications related to your water assessment
• Water quality education information

Message frequency: Varies based on your registration and appointment status.
